How to Refer Patients or Students
We are grateful to those who take the time and effort to refer for developmental vision care!
- If you are a referring physician, your front office staff is welcome to call our office at 913-469-8686 to schedule an appointment when your patient is at your office.
We will then contact your patient to prepare them for the evaluation in our office.
- If you are a referring school professional, please complete and share the Teacher/OT referral checklist (PDF) and give it to the student's parent or guardian. Then it's up to them to consider contacting our office to schedule an appointment.
When a patient is referred and lets us know, we will send you a vision report and keep you updated on your patient or student's plan of care.
