Vision Therapy — Insurance & Payment Policies
The following is an explanation of our payment guidelines and insurance policies. We are happy to answer any questions regarding our procedures.
Payment Schedule
The care of our patients is paramount, and we strive to provide quality care for fair and reasonable fees. All patient services are to be paid-in-full as services are provided. If you have insurance, we will file a claim and you will be reimbursed directly as your plan permits. If for some reason an account should accrue a balance, we reserve the right to charge a service fee to accounts in arrears.
Health Insurance
We are an optometry practice providing a special service which requires a significant amount of time and attention to ensure a high quality of care for every patient. The amount of time we spend with each patient is not typical for general optometry practices. For this reason, we can not afford to participate in most insurance plans.
For our patients’ convenience, we are happy to file insurance claims and will do everything we can to assure proper reimbursement. We will also be happy to prepare and submit a letter of pre-approval for services, accompanied by documentation of medical necessity and a detailed report (if in-depth testing has been completed).
We cannot take responsibility for services not covered by health insurance. Costs associated with patient care are expected to be paid as incurred. Here are the ways we can help each other:
Patients can:
- Call your insurance company to determine your coverage. We are happy to give you procedure and/or diagnosis coces to aide in your research.
- Obtain insurance claim forms, if needed, from your agent.
- If the treatment is a result of an on-the-job injury, determine company coverage, and present proof of filing information.
- Authorize our center to furnish information directly to your insurance company to expedite handling of claims.

Appointments
Because appointment times are reserved specifically for patient’s preferred times, we ask that we are notified at least 24 hours in advance when canceling or rescheduling an appointment. This courtesy allows us to maintain a high level of care for other clients as well. Thank you for your cooperation in helping us to serve everyone as efficiently and effectively as possible.
